RFP Regarding Health Disparities

The Minnesota Department of Human Services (DHS), is seeking proposals from qualified Responders to conduct an environmental scan and data analytics which will identify Minnesota’s Medicaid populations who experience the greatest health disparities and require greater investment from providers to meet their needs.  These ‘target populations’ will be identified by their social risk factors such as health conditions (eg mental illness and chemical dependency), lack of resources (eg homelessness), and language and immigration factors. DHS is also seeking proposals from qualified Responders who will conduct an environmental scan and data analytics to develop a payment methodology which takes into account the proportion of a provider’s patients that belong to one of the targeted populations, as well as the provider’s ability to achieve better performance for these populations.  Responders can submit proposals for either or both components of the RFP.  See the posting at: http://www.mmd.admin.state.mn.us/process/admin/ptList.asp
